NO WEEKENDS - WORK MONDAY THROUGH FRIDAY 8am - 4:30 pm General Statement of Duties: The Nurse Advocate, under the supervision of the Clinical Manager for day-to-day direction and Chief Medical Consultant for clinical supervision, is primarily responsible for maintaining the best possible health for all VMRC consumers. Will be familiar with the protocols used by VMRC to assess consumer access to health care and assist in preparing or modifying protocols as necessary. Will have assigned a specific consumer caseload and one or more case management units for primary consultation.
Working Condition and Physical Requirement: Majority of duties are performed in the office. The ability to drive an automobile is required on a daily basis. The Nurse Advocate must maintain a valid driver's license and be able to travel locally and out-of-town regularly, including occasional overnight stays and attend evening and weekend meetings. Travel throughout the five-county area will be required as workload demands. The job duties require the ability to sit or stand for long periods of time, and to operate office machines requiring manual dexterity and repetitive key entry.
The ability to read, write and comprehend large amounts of written material, use the telephone, and communicate via electronic mail are required on a daily basis. Excellent oral and written skills are essential.
Responsibilities:
1. Review client records at VMRC, the records of service providers contracted with VMRC, and other necessary and available data to verify that specific consumers have the following necessary components to good health care: a) a satisfactory data base; b) adequate access to health care providers; c) appropriate provision of medical and dental care consistent with medical needs; and d) appropriate supervision of health care needs.
2. Provide primary clinical consultation to the Program Manager and/or Service Coordinator within the units that have been assigned.
3. As required, visit day programs, residential facilities, hospitals, skilled nursing facilities, intermediate care facilities and any other programs or institutions where VMRC consumers are served, to insure that consumers? special health care needs are met and to identify any potential hazard to the well-being of consumers.
4. Provide health and wellness related advice on request to clients, families and other interested individuals.
5. Facilitate liaison between case management staff and clinical services, including clinics, consultant physicians and therapists, etc.
6. Attend clinical staff meetings and other meetings as assigned.
7. Maintain clinical skills by attending seminars, workshops and other continuing education offerings.
8. Other job-related duties as assigned.
Minimum Position Requirements: A.A. degree in Nursing, one year nursing experience and California licensure as a registered nurse. Knowledge of nursing principles and equipment applicable to serving the developmentally disabled; consultative techniques; community organizations and resources; developmental process from conception; and health and disease aspects of diagnosis and treatment. Ability to train and teach professional and lay persons, relate to families under stress, and individuals and groups providing related health care. Ability to read, understand and explain medical records and notations. Ability to work in a high performance, team-oriented environment.
