JOB DESCRIPTION:
Community Health Nurse Supervisor
General Description: A fulltime nursing position with split responsibilities performing
outpatient clinical nursing duties and supervision of community health outreach
program performed by Grand Aides (clinically advanced community health workers).
Responsibilities:
Grand Aides supervision (50%)
· Participate in recruitment and hiring of Grand Aides to work in a community setting.
· Train Grand Aides in required clinical skills, including specific triage protocols for
working with patients in the field and appropriate documentation.
· Supervise Grand Aides during their apprenticeship in the primary care clinic and
provide support and oversight as they transition into community care activities.
· Assist in recruiting appropriate patients for Grand Aides services and link
to assigned Grand Aides staff.
· Provide telephone advice for home visits performed by Grand Aides
as appropriate and as guided by Triage Protocols and provider/RN clinical judgment.
· Review and co-sign all Grand Aides documentation.
· Provide continuing education and support for Grand Aides in the field.
· Support Grand Aides work as a liaison for important and interrelated
preventive, acute and tertiary services and resources as indicated for management
of patient and family overall well-being.
· Collect data related to Grand Aides service provision and patient satisfaction.
Primary Nursing Responsibilities (50%)
· Assist with OSHA compliance tasks and infection control.
· Perform medical procedures according to the standard Legacy Community Health Services protocols.
· Administer drug treatments according to standard protocols and physician orders.
· Assist the Nurse Manager in the ongoing assessment of clinic services in the clinical and
administrative areas and report any need for improvement in those areas.
· Maintain an ongoing inventory record for medications and order medications indicated.
· Perform phlebotomy and other diagnostic medical tests, such as Gram Stains, etc. as appropriate.
· Document all client care as appropriate in a timely manner.
· Assess, treat, and provide counseling to clinic clients.
· Consult with clients by telephone, triaging and referring these clients to services as appropriate.
· Assist physicians as needed.
· Evaluation of lab results with follow-up of abnormal values as appropriate.
· Participate in the after hours on-call program, triaging calls, consulting with physicians
and providing appropriate documentation of all client contacts.
· Assist Nurse Manager with chart audits and Q/A as assigned.
· Some evening and weekend hours may be required prn.
· Other duties as assigned.
Requirements:
1. Current Texas RN license required.
2. Over 5 years nursing experience in the primary care setting.
3. Experience with supervisory roles.
4. Clinical teaching experience preferred.
5. Familiarity with local community preferred.
6. Ability to communicate easily with others.
7. Bilingual Spanish/English a plus.
